What is it and what does it involve?

Surgery to correct protruding ears, known as otoplasty, must be tailored to the needs of each patient and meet specific anatomical and aesthetic goals. A complete, thorough examination of each person is therefore needed to assess the deformities causing the problem.

Pre- and post-operative photographs are also essential to plan and document the surgery properly. It is equally important to explain to the patient the goals to be achieved, so that they know in advance what results can be obtained.

Its aesthetic and psychological impact can be very severe, which is why its surgical correction, otoplasty, is widely accepted and well tolerated in both children and adults. The aim of this surgery is aesthetic, and it is important that the surgeon has a comprehensive knowledge of the anatomy. An attractive ear depends on its position, size and the harmony between its parts, and it is one of the most varied parts of the body. Generally, the ear has soft, graceful curves and should follow the shape of the head without sticking out excessively, with symmetry in the projection of its different parts.

The most common deformity is the so-called hypoplastic antihelix, where the ear has folded less on itself and the outer ear sticks out more than normal. This can result from embryological development or excess cartilage, among other causes. The earlobe may also be prominent or over-projected.

The surgical techniques used in otoplasty fall into two types: those that cut the cartilage and those that reshape or preserve it. The first involve resection and partial or full-thickness incisions, and generally produce longer-lasting surgical results. Reshaping, on the other hand, uses sutures or implants to recreate normal ear anatomy. This technique avoids scarring or fibrosis but has higher recurrence rates.

In recent years, a combination of both techniques has been used for otoplasty. Complications from this surgery are uncommon and it is generally well tolerated. In adults it can be performed under local anaesthesia without a hospital stay; in children, sedation is normally used.
